Daytime care of any part of a day but less than 24-hour care for functionally and/or cognitively impaired older adults provided through a structured program of social and rehabilitative and/or maintenance services in a supportive group setting other than the client's home.

The provision of nutritious meals to homebound older adults Monday through Friday.  Up to 2 meals per day may be provided to most eligible older adults.  However, the following additional meals may also be available where certain conditions apply:

In-home Supports provides assistance with essential services that allow older adults to remain independent at home.  Essential tasks* include assistance with the Activities of Daily Living such as:

• Bathing
• Dressing
• Bathroom assistance
• Shopping for essential items
• Errands

In home respite care is also available for older adults with family caregivers that allows the caregiver to leave the home for brief periods for a break

*Basic housekeeping tasks are only available as part of an overall care plan that supports essential tasks.

Personal Emergency Response Systems (PERS) for frail older adults with no informal support system in place (family, friends, etc). Clients must meet the following eligibility criteria:

• Client must be oriented and in possession of faculties to absorb training in the equipment and be able to operate the button appropriately.
• Client must be homebound for a no cost PERS unit OR at least 2 of the following must be true:
  • Client has fallen at least once in the 90 days
  • Client has been hospitalized in the last 90 days
  • Client lives alone with NO family contact

 Additional information:

• GPS units are available to non-homebound clients for a monthly fee of $25.00
• Clients living in housing units with safety pull cords are not eligible for the no cost PERS units; however, they are available at the contracted (lower) rate

Older adults who are no longer comfortable driving can access this service for medical and legal appointments, grocery shopping and to run errands. Depending on the mobility level of the senior, service can be provided on a wheelchair friendly bus or with a volunteer driver. Rides are prioritized using the following criteria:

• Medical appointments
• Nutritional appointments
• Legal appointments
• Shopping and errands

Door-to-door service is provided by volunteer drivers. Please allow 3-5 business days advance notice for scheduling.

Volunteer Senior Transportation is available through Allegan County Transportation. Depending on mobility of the senior, rides will either be offered by a volunteer driver or a transportation bus with lift.
