About the Program

Allegan County Community Links is about bridging the gap between individuals, communities, and the Health and Social Service Systems. We work to connect community members through referrals to get the services they have difficulty locating within the community.

What We Do

A Community Health Worker serves as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

Our Allegan County Community Health Worker provides referrals, information on community services, and assistance with receiving services to residents of the community. Some areas we work to link you to include:

  • Food/Meal assistance
  • Primary Medical and Dental care
  • Clothing and furniture
  • Utilities
  • Transportation
  • Housing
  • Education
  • Financial Education and Resources
  • Support groups
  • Domestic Violence education, case management, SA therapy, and therapy for children who have witnessed abuse
  • Support and treatment for substance abuse/mental health issues
  • Crisis Intervention
  • Legal

Eligibility

We are currently working with all residents/members of the community.

Those experiencing certain situations outlined below will be our top priority.

  • You are eligible for Medicaid
  • You have a pending Medicaid application approval
  • You are not currently receiving ACCL services already
  • You do not have a primary care provider
  • You currently have Medicaid
  • You are experiencing homelessness
  • You are experiencing food insecurity
  • You are facing domestic violence-any gender
  • You are seeking Mental Health Resources
  • You are in need of immediate referral/services assistance*
  • You do not feel safe at home

*Immediate need of referrals/services would be you/family are homeless, no food at home no ability to obtain food without assistance, will/have been evicted, facing domestic violence and seeking a shelter, in need of mental health services/substance abuse/counseling. Please call 9-1-1 if immediate emergency assistance is needed/required as Community Health Workers only provide/seek referral services.
