Become a COVID-19 Vaccine Provider

We need your help.
Currently about 3,000 providers are eligible and have not yet enrolled to become a “COVID-19 Ready Provider” to administer the COVID-19 vaccines. Since we know that people — especially parents — trust their own doctor or health care provider most for vaccine information, you play a critical role in answering parent questions, administering the vaccine and helping to protect adolescents from COVID-19.
 
All currently vaccinating health care providers registered in Michigan Care Improvement Registry (MCIR) to provide immunizations must also enroll in the CDC COVID-19 Vaccination Program to be able to receive and administer COVID-19 vaccines.

Enroll.. it's as easy as 1-2-3!
To enroll as a “COVID-19 Ready Provider” and ensure that your office is ready to administer vaccine for adolescents and adults, ages 12 and older, complete the following steps:

Providers must be a registered MCIR user. To register, complete this form and return it to the appropriate regional office. If you are already a registered MCIR user, skip this step.

Once registered with MCIR, fill out and submit the CDC COVID-19 Vaccination Program Provider Agreement for the practice.

The individual completing the CDC COVID-19 Vaccination Program Provider Agreement will be receiving all emails regarding vaccines from the CDC and VaccineFinder. Once submitted, a confirmation email will be sent, and the CMO and CEO or the practice owner will receive a link to sign the agreement.

Finally, log into MCIR (through MILOGIN) to complete the CDC COVID-19 Vaccine Site Provider Registration. The Corporate/Organization CMO License number will be needed to complete registration.
