COVID-19 Isolation: "I tested Positive"

Self-Reporting Forms
To help achieve timely and efficient reporting, we are asking Allegan County residents only (meaning that you live in Allegan County) that test positive to complete the self-reporting information below. This is a safe and secure link to provide ACHD with your information.

What to do if you test positive:
- If you test positive for COVID-19 or have symptoms, regardless of vaccination status, stay home and isolate for at least 5 days* (View the CDC Calendar at the bottom of the page to help you determine your isolation timeframe).
- View the updated CDC Isolation Guidance (updated March 2022)
- To keep others safe in your home, wear a mask, stay in a separate room and use a separate bathroom if you can. If you can’t wear a mask, stay home and away from other people for 10 days.
- On day 6: Do a self-check. How are you feeling? You could have loss of taste or smell for weeks or months after you feel better. These symptoms should not delay the end of isolation.
- If you have no symptoms or symptoms improving. No fever without fever-reducing medication for 24 hours: You can leave isolation. Keep wearing a mask around other people at home and in public for 5 more days (days 6-10).
- If your symptoms are not improving and/or still have fever: Continue to stay home until 24 hours after your fever stops without using fever-reducing medication and your symptoms have improved. After you feel completely better, keep wearing a mask around other people at home and in public through day 10
*Day 0 is the day you were tested if you don’t have symptoms, or the date your symptoms started.

- Notify your Close Contacts
- ACHD asks individuals who test positive to notify their close contacts. Individuals can use tellyourcontacts.org, to help notify close contacts anonymously.

- Test to Treat Location Finder: Through this program, people are able to get tested and – if they are positive and treatments are appropriate for them – receive a prescription from a health care provider, and have their prescription filled all at one location.

Accessing Care during Isolation:
If you have a medical emergency during your isolation period and have to call 911, please tell them you are currently in isolation due to having COVID-19. If you need to see a health care provider during your isolation period, call ahead to let them know your situation.
