Your feedback is important to us. Following are ways to connect and let us know how we're doing.
  • Inquiry - report a health concern or submit a question around sewage, food establishment, health, or other. View instructions on filling out the Inquiry Form.
  • Media Request - for media personnel to fill out when seeking information on a specific topic, or an interview with Health Department staff.
  • Referral Survey - for Allegan County Community Links program that guides community members to local resources to help meet their unmet needs. Learn more about this program.
  • Feedback Survey - for rating client satisfaction; select "Health Department" from Service Areas on the form
